Re: Illinois Freedom of Information Act Request

Pursuant to Illinois’ Freedom of Information Act, 5 ILCS 140, and on behalf of Alexi for Illinois, I request the following documents, information and data:

· The list of all registered voters in your jurisdiction, including voters marked as inactive.

· The name of all individuals (and all data associated with each individual) (a) who have requested absentee ballots, including those individuals on any permanent absentee voter list; (b) who have been mailed an absentee ballot by the election authority/Board; (c) who have returned an absentee ballot to the election authority/Board; (d) whose absentee ballot has been deemed by the Board to be eligible for counting, and/or; (e) whose absentee ballot has been deemed ineligible for counting. For each such individual, please also provide the election authority/Board’s reason for rejecting the ballot.

· Copies of all absentee ballot envelopes (e.g., the envelope containing the voter’ signature) received by the election authority/Board. The copy should include a visible image of the voter’s signature.

· The name of all individuals (and all data associated with each individual) who have submitted (or on whose behalf has been submitted) voter registration applications, and whose applications have been rejected by the election authority/Board.

· The names of all individuals (and all data associated with each individual) who voted either by Regular or Provisional Ballot during the early vote period. Please indicate whether each voter cast a regular or provisional ballot, and if the voter cast a provisional ballot, the reason(s) for doing so.

· The names of all individuals who voted by Provisional ballot during the early vote period, and whose ballot has been deemed by the election authority/Board to be ineligible for counting. For each such individual, please also provide the election authority/Board’s reason for rejecting the ballot.

· Copies of all provisional ballot envelopes completed by voters during the Early voting period and on election day.

· The paper pollbook generated for each of the polling places for Election Day.

· The supplemental list of early voters, the supplemental list of absentee
voters, and the administrative challenge list, and any other supplemental lists generated for each polling place for Election Day.

· The name of all individuals (and all data associated with each individual) who voted a Provisional Ballot on Election Day, along with the reason why the voter was not allowed to cast a regular ballot, and if the ballot was not counted, the reason for not counting the ballot.

· Copies of any documents or certificates required to be completed or signed by the election judges on Election Day

· Copies of the “Statement of Ballots” form completed by election judges in each precinct on Election Day

· Copies of the exterior of the “Before 7:00 P.M. Envelope” and the “After 7:00 P.M. Envelope” from each precinct

· Copies of the logs maintained pursuant to 10 ILCS 5/24B-13 and 5/24C-14

· Copies of all lists of absentee voters maintained pursuant to 10 ILCS 5/19-4

· Copies of all lists of early voters maintained pursuant to 10 ILCS 5/19A-35(b-10)

· Copies of the lists of Early Voters, Grace Period Voters, and Absentee Voters delivered to each precinct on Election Day

· Copies of the notices sent to rejected absentee voters sent pursuant to 10 ILCS 5/19-8(g-5)

If documents, information or data can be e-mailed, you may email them to xxxx@xxx.com. If documents, information, and data cannot be emailed, please let us know, either via e-mail or phone and we will send someone to your office to pick them up. If you anticipate being unable to comply with any part of this request within 7 days, please let us know immediately. Please indicate whether, despite being unable to comply with part of the request, you are still able to comply with other parts of the requested. Please inform us of any charges associated with these request and the method of payment requested.
